Club Parent/Carer Declarations

 

I/We give my/our consent to my/our child/taking part in football training and playing activities arranged and

overseen by Club managers, coaches, officials and/or helpers of The Basingstoke School Of Development Centre Youth Football Club.

 

I/We understand that the team selection rests solely with the manager, and participation in any Basingstoke School Of Development Centre Youth Football Club activity is at our and our child’s own risk.

 

I/We understand responsibility for supervision of players will only be assumed by club managers, coaches, officials and/or helpers of The Basingstoke School Of Development Centre Youth for the period of matches and training only.

Parents/Carers must remain with their children at all times or arrange for another responsible adult to assume responsibility for their child/children. Parents/Carers leaving their child before or after such sessions retain responsibility even in their absence.

 

I/We agree to our child receiving first aid treatment as considered necessary by any Basingstoke School Of Development Centre Youth Football Club first aid trained manager, coach, official or helper and any emergency dental or medical treatment as considered necessary by any medical authorities present.

 

I/We understand that whilst the The Basingstoke School Of Development Centre Youth Football Club  managers, coaches, officials and/or helpers in charge of our child will take all reasonable care, they cannot be held responsible for any loss, damage or injury suffered by our child/children  travelling to or from, the destination in Holland  or taking part in any games or training sessions.

 

I/We agree in the event that my child/children is/are injured whilst playing football/travelling to and from the destination in Holland and  football events and if  I/We cannot be contacted on the contact and emergency numbers provided , I/We hereby give consent for my/our child to receive medical attending.

 

I/We understand the extent and limitations of the insurance cover provided by the club being Public Liability Insurance only.

 

I/We understand that  The Basingstoke School Of Development Centre Youth Football Club or any individual in the Club are not liable for loss or damage to any property arising out of Club activities.

 

I/We I agree to pay any fines imposed on the club by the Hampshire FA or any other relevant governing body as a result of disciplinary action against me or my child.

 

I/We have read and agree to adhere to The Basingstoke School Of Development Centre Youth Football Club Safeguarding Children Policy, Codes of Conduct and Equality Policy, which have been made available to us by email.

 

I/We understand that if our child or I/we act in contravention of The Basingstoke School Of Development Centre Youth Football Club Parent/Carer Declarations Statements, Club Rules, Codes of Conduct or Safeguarding Children Policy, I/we may be liable to suspension or expulsion from the club.

 

I/We agree to be bound by and to observe all of The Basingstoke School Of Development Centre Youth Football Club Parent/Carer Declarations Statements and Club Rules (please see below) and The Rules and Regulations of the Football Association Limited and all Competitions in which the club participates.

 

I/We confirm that the contact and any medical details which have been given are correct.

 

I/We agree that The Basingstoke School Of Development Centre Youth Football Club or their representatives may take photographs/filming’s for promotional purposes only which I am happy for my child/children taking part in.

 

I/We can confirm that we have taken out relevant travel insurance for each and every member attending this tour as advised to do so by The Basingstoke School Of Development Centre Youth Football Club. If I/we have not taken such cover I/we accept that The Basingstoke School Of Development Centre Youth Football Club will not be liable to pay for any medical expenses incurred by you or your child/children.

 

 

Club Rules

 

  1. The Basingstoke School Of Development Centre Youth Football Club Players and Parents must complete the online registration for each child, detailing all contact details and any known medical issues/illnesses.
  2. Remember The Basingstoke School Of Development Centre Youth Football Club players, parents, carers and spectators are ambassadors for our club.
  3. At no time will The Basingstoke School Of Development Centre Youth Football Club players fight or argue between themselves or others.
  4. The Basingstoke School Of Development Centre Youth Football Club Players and Parent/Carers are personally responsible for any fines/administration fees levied against them by any League or Football Association.
  5. Any The Basingstoke School Of Development Centre Youth Football Club player or parent/carer who does not pay fines/administration fees levied against him will be suspended from The Basingstoke School Of Development Centre Youth Football Club until the fines/administration fee is paid.
  6. The Basingstoke School Of Development Centre Youth Football Club players are responsible for their own belongings.
  7. The Basingstoke School Of Development Centre Youth Football Club players must wear kit provided by the club, not different colour socks, shirts, shorts etc.
  8. The Basingstoke School Of Development Centre Youth Football Club players must wear shin pads at all matches and training sessions. (FA Rule)
  9. The Basingstoke School Of Development Centre Youth Football Club players must tuck their shirts into their shorts at all times while participating in a match.
  10. The Basingstoke School Of Development Centre Youth Football Club players must always play to the Referees whistle and never argue with the Referee.
  11. No arguing with team managers. The manager’s decision is final.
  12. All jewellery must be removed for matches and training. (FA Rule)
  13. Players and parents/carers must abide by The Basingstoke School Of Development Centre Youth Football Club Codes of Conduct, Safeguarding Children Policy set out by the Club.
